Our goal for the Life Choice Society is to provide Options for Women PHC with a stable base of operating revenue for a five-year period. This provides a stronger foundation for expanding the reach of our services, while promoting healthier lives!
Through Options for Women the cost to provide our services is $1,200. We encourage you to make an investment in the lives of the women and men in Polk County and their futures. You can choose from the following levels of our Life Choice Society, with your pledge being monthly or annually. Please be sure to select a recurring gift!
Choice Level – Support Lives with $1,200 /year ($100/month) for 5 years
Empowerment Level – Support Lives with $2,400/year ($200/month) for 5 years
Freedom Level – Support Lives with $3,600/year ($300/month) for 5 years
Foundation Level – Choose the amount to give per month/year for 5 years
Our new clinic on South Florida Ave. will allow us to serve 3-4x more patients.
In 2023, 436 lives were saved at our clinics. Can you help us increase this number in the coming years by donating funds toward the construction of our new clinic?
There are a variety of ways the community can help us serve our patients, in addition to volunteering and donating directly to us. Below is a wish list for each of our departments. If you can help us with any of these items, please call our office at (863) 682-5763.
Patient Resources:
- $25 gift cards to Walmart for patients who decided to parent and return with their babies 9 months later
- Bus passes to provide to patients who need transportation assistance
- Sponsorship for radio ads to reach our demographic
- Sponsorship for ads on college websites
- Sponsorship of print materials to distribute on college campuses and the community
- Media allowance for demographic tracking
- Underwriters for staff to attend professional development conferences
Medical Services:
- Educational and travel scholarship for organizational conferences
- Scholarship for sexual integrity training events
- Pregnancy tests
- Prenatal vitamins
- Seabands for patients with nausea
- Sonography and OB resource books, manuals and videos
- Scholarship for a new nurse sonographer
Administration:
- Printer Toner (TN 760 Brother Toner)
- Flat screen LCD computer monitors
- Underwriters for fundraising events
- Underwriters for upgraded database software
- Tent for outdoor community events
- Underwriters for professional development workshops and conference
